Your care team will provide personalized services, conjointly by an integrated multidisciplinary team, whose members respect the values, preferences, and expressed needs of client and families. Your dedicated Care Manager will provide information outlining your health condition, proposed treatment and procedures along with the risks and benefits, and any alternate courses of treatment in terms and language that can be understood by you. Please let us know if you have any request orsuggestions. This binder explains the services tailored to your specific needs. Your home health team will routinely visit you until you can safely manage to care for yourself or with the help of the family member. Please have this binder available every visit.
